Meat Collective Student Forms

Students will have to do a bit of paperwork before they can pick up a knife for your Meat Collective classes. These templates will give you a sense for the kind of paperwork you'll need to provide your students before class starts.  As always, these are meant as examples only. Consult with lawyers within your state and/or your state's department of agriculture to ensure the language in these templates abides by you state's laws and regulations.

Download includes:

"Waiver and Release of Liability (including coverage for owner of premises)" Template: Make sure you and your host site are protected in case anything goes wrong during class. 

"Acknowledgement of Receipt of Slaughtered Animal for Personal Consumption" Template: If the meat you and your students purchase for use in class was slaughtered under custom-exempt regulations, recipients will have to fill out something like this. If you're using USDA-inspected animals, you won't need this, although you could use it anyway, just in case. Consult our Meat Collective Startup Guide for more information on these subtleties.
